This driver performance scorecard template for Excel is a free Excel template for recording monthly safety, service, fuel, inspection, and customer measures by driver. It supports one record for each driver and reporting month, then summarizes results for the fleet and a selected driver.

Across its three sheets—Dashboard, Driver Scorecard, and Settings—the file separates entries from formulas. Yellow cells identify input cells and editable selections, while blue cells show calculated outputs. Populated sample rows illustrate the intended format. Prepared blank operating rows below the initial sample area let users continue entering monthly records without replacing the existing formulas.

How to use the file

Build the driver and reporting lists

Begin on Settings by reviewing the populated examples and replacing or extending the operating lists. Enter driver IDs and names in D4:E103, reporting periods in H4:H103, regions in J4:J7, and review statuses in M4:M6. Those ranges provide the dropdown choices used elsewhere in the file. Reporting-period dates are validated from January 1, 2020, through December 31, 2035.

Confirm weights, targets, and cutoffs

Before entering operating data, review the yellow assumption cells in Settings column B. Cells B4:B8 hold the five score weights and must total 100%. B10:B12 contain the on-time, MPG, and inspection targets. B13:B15 hold penalty assumptions, B17:B19 contain rating cutoffs, and B21:B22 contain suggested bonus values.

Cells B24:B27 are outputs rather than entries. B24 totals the weights, while B25:B27 test the weights, cutoff order, and other assumptions. Dashboard displays Settings Check as OK only when all three checks return OK; otherwise, it displays Review Settings.

Record one driver for each reporting month

Next, use Driver Scorecard columns A, B, D:O, AC, and AD for monthly input fields. Enter the reporting period, driver ID, region, miles driven, fuel used in gallons, total stops, on-time stops, required inspections, completed inspections, speeding events, harsh braking events, preventable accidents, customer complaints, supervisor notes, review status, and follow-up date.

Leave Driver Name in column C and columns P:AB unchanged because they are calculated columns. Their formulas return MPG, on-time rate, inspection rate, safety events per 1,000 miles, five component scores, overall score, rating, suggested bonus, and data check. The initial DriverData table occupies A4:AD14 and contains populated sample records. Formulas and validation ranges continue through row 1000, including prepared blank operating rows below that initial table range. A note on the sheet instructs users to press Tab from the final table cell to add a new Excel table row.

Review a reporting period on Dashboard

Finally, choose a Reporting Period in Dashboard cell B4 and a Selected Driver ID in E4. Both yellow control cells use lists supplied by Settings. The driver name in F4 is calculated from the selected ID rather than entered manually.

Most fleet totals and scored performance results use only matching records whose Data Check result is OK. Check the Data Issues count before interpreting the period summary or comparison results.

Calculate monthly driver performance measures

For every row containing both a reporting period and driver ID, formulas calculate MPG by dividing miles driven by fuel used, on-time rate by dividing on-time stops by total stops, and inspection rate by dividing completed inspections by required inspections. Safety events per 1,000 miles combine speeding and harsh braking events before dividing by miles and multiplying by 1,000.

Each safety, on-time, fuel efficiency, inspection, and customer service component score is limited to a range of 0 to 100. The Overall Score applies the five editable Settings weights. A separate formula compares that score with the Excellent, Good, and Needs Improvement cutoffs; results below the Needs Improvement cutoff receive a Poor rating.

Find driver scorecard data issues

Validation supports consistent entries across the prepared rows. Reporting periods, driver IDs, regions, and review statuses use lists. On-time stops must be numeric, zero or higher, and no greater than total stops. Completed inspections follow the same relationship with required inspections. Follow-up dates accept dates from January 1, 2020, through December 31, 2035.

Beyond cell validation, the Data Check formula can return messages for a missing period or driver, an invalid driver ID, a duplicate driver and reporting-period combination, or required activity that needs review. Dashboard Data Issues counts records in the chosen reporting period whose result is not OK. Correcting those source entries allows the dependent formulas to recalculate.

Compare fleet and individual driver results

Fleet Snapshot shows Drivers Reported, Average Overall Score, Fleet On-Time Rate, Fleet MPG, Total Miles, Safety Events per 1,000 Miles, Preventable Accidents, and Suggested Bonus Total for the selected reporting period. Ratio outputs use the applicable totals rather than simply averaging each underlying row percentage.

Individual review fields include overall score, rating, on-time rate, MPG, inspection rate, safety score, customer complaints, and review status. Driver Comparison lists driver ID, driver name, overall score, rating, on-time rate, MPG, safety score, and data status. The included chart references driver names and overall scores displayed in Dashboard rows 26 through 33.

Adapt scoring assumptions to fleet policy

Editable Settings values control the relative weight given to safety, on-time service, fuel efficiency, inspection completion, and customer service. Managers can also revise the performance targets, penalties applied in the score formulas, rating thresholds, and two suggested bonus amounts without rewriting those formulas.

Three calculated checks identify weights that do not total 100%, rating cutoffs that are not in the required descending order, or assumptions that fall outside the tested conditions. The Suggested Bonus assigns the B21 planning amount to an Excellent rating and the B22 amount to a Good rating; other ratings return zero. These figures are planning estimates, not payroll instructions.
