The driver equipment issue and return log keeps a structured custody history for gear assigned to drivers. This free Excel template records issue dates, equipment, quantities, vehicle or unit details, conditions, due dates, returns, damage, loss, estimated exposure, and actual charges.
Four connected sheets cover daily entry and review: Dashboard, Issue & Return Log, Equipment List, and Settings. The log contains 999 prepared rows from row 2 through row 1000. Populated examples in the early rows are sample records to review or replace; unpopulated rows are blank operating rows with formulas and validation already prepared. Calculated fields should remain intact while users enter data in the designated input columns.

How to use the file
Set the operating date and reference lists
Begin on Settings. Update the As of Date, general default return days, due-soon window, exposure rates, damage estimate rate, and maximum quantity per log. The As of Date drives overdue calculations and Dashboard reporting. Maintain the Driver ID, Driver Name, Active, receiver, condition, active-choice, and category lists used by the workbook.
Define the equipment catalog
Use Equipment List to enter up to 100 catalog rows. Assign a unique Equipment ID, then enter Category, Description, Brand / Model, Asset Tag / Serial No., Default Return Days, Replacement Cost, Active, Storage Location, and Notes. Equipment IDs feed the log selection list. Description, category, default return days, and replacement cost are retrieved from this catalog.
Enter an issue transaction
On Issue & Return Log, enter a unique Log ID, Issue Date, Driver ID, Equipment ID, Vehicle / Unit, Odometer at Issue in miles, Issued Qty, and Condition Out. Due Date Override is optional. Driver Name, Equipment Description, and Category are calculated from the selected IDs. The calculated Due Date uses the override when entered; otherwise it uses the issue date plus the equipment default, or the general default if an equipment-specific value is unavailable.
Post return or loss details
When equipment comes back, enter Return Date, Returned Qty, Condition In, Damaged Qty, Lost Qty, and Received By. Actual Charge Amount and Notes are also user-entered fields. Formulas calculate Outstanding Qty, Days Out, Days Past Due, Estimated Exposure, and Status. Possible statuses include Issued, Partially Returned, Overdue, Returned, Returned - Damage, Lost / Review, and Check Entry.

Record driver equipment custody at issue
Each log row connects an issue event to a driver and an equipment record. Dispatchers can record the vehicle or unit, odometer at issue in miles, quantity, and outgoing condition without retyping catalog details. Selecting Driver ID fills Driver Name, while selecting Equipment ID fills Equipment Description and Category.
The Log ID validation checks for duplicates across the prepared range. Issue quantity must be a whole number from 1 through the maximum quantity set on Settings. Odometer values must be zero or greater.
Review open, overdue, and upcoming returns
The Dashboard reports total log records, open records, open units, overdue open records, quantity return rate, estimated exposure, and actual charges. Due Within Window counts open records due between the As of Date and the number of days selected in Settings.
Category summaries show issued, returned, and outstanding quantities, overdue records, and exposure. Status summaries show record counts, estimated exposure, and actual charges. The included chart compares outstanding quantity by category, while red or orange formatting identifies items requiring attention.
Estimate equipment exposure without treating it as a posted charge
Estimated Exposure is calculated from replacement cost and the open, lost, and damage rates on Settings. Outstanding quantities use the open equipment value rate, lost quantities use the lost equipment value rate, and damaged quantities use the damage estimate rate.
These values are planning assumptions. Actual Charge Amount is entered manually for an approved or posted amount and is not calculated from exposure. This separation lets managers compare a working estimate with the amount actually recorded for the event.
Identify incomplete or inconsistent return records
Dashboard data checks count duplicate Log IDs, Check Entry records, returns missing Received By, and quantity rule violations. A record can be marked Check Entry when required information is missing or quantities and dates conflict.
Quantity logic prevents returned plus lost quantity from exceeding issued quantity and prevents damaged quantity from exceeding returned quantity. A return with a positive returned quantity also requires a Return Date and Received By entry. Filters in the Excel tables can then be used to isolate the records that need review.
Common questions
About this workbook
Yes. Issued Qty can contain more than one unit, subject to the maximum quantity set on Settings. Returned Qty and Lost Qty divide that issued quantity, while Damaged Qty identifies the returned units reported as damaged. Use separate records when individual asset tags or separate custody events must remain distinct.
If Due Date Override is blank, Due Date uses the equipment's Default Return Days. If that lookup is unavailable, the formula uses General Default Return Days from Settings. An entered override replaces that calculated schedule and must be on or after the Issue Date.
Issue Date accepts dates from January 1, 2000, through the current As of Date in Settings. Return Date may be blank, but an entered date cannot be earlier than Issue Date or later than the As of Date. These controls depend on keeping the As of Date current.
No. Active is available in the driver and equipment reference data, but the log validations reference the full prepared Driver ID and Equipment ID ranges. The manifest does not show formulas that filter those selections by Active status, so list maintenance remains a user task.
No. It is an operational custody and planning workbook. Estimated exposure is not an invoice, payroll deduction, or accounting entry. The file does not replace accounting, legal, tax, DOT/FMCSA, safety, or compliance advice, and it does not provide integrations or external reporting.
